Refer a Patient
Online Referral Form – Fill Out Online
Submitting a referral is simple. Complete the online form with your patient’s information, the reason for referral, and any relevant history. Including recent bloodwork, anesthetic records, and dental radiographs helps us plan efficiently and may shorten anesthesia time.
How We Support Your Practice
We believe in nurturing the human-animal bond and the relationships between pet owners and the veterinarians who care for them. Dentistry is our focus, and we rely on you to continue overseeing your patients’ wellness and medical care. After every consultation and procedure, we share our findings, radiographs, and recommendations so you have the whole picture.
Dr. Martin and her team prefer to work from our Beaverton office, but she can travel to your hospital on request. When your clients have a wellness plan that includes a dental cleaning, we can coordinate a staged approach — supervising or reviewing your findings and x-rays — so treatment decisions are made thoughtfully rather than under time pressure.
Cases We Commonly See
We accept referrals across all six disciplines of veterinary dentistry, including periodontics, endodontics, restoratives and prosthodontics, oral medicine, orthodontics, and oral surgery. Common referrals include fractured teeth for root canal therapy, feline stomatitis, oral masses, jaw fractures, malocclusion, and cleft palate repair.
Your clients can expect a courteous welcome, clean exam rooms, a friendly doctor, and caring technicians. We appreciate the role we get to play in your patients’ health care. View our full list of conditions treated.
